We understand that once you’re feeling better, you’ll want to return home as soon as it’s safe to do so. That’s why we’ll work with you to plan your discharge together, helping you get back to the people, places, and routines that matter most – whether that’s enjoying your own bed, making a cup of tea, spending time with loved ones, or caring for a pet.
Being at home is often the best place to recover. Research shows that patients continue to get better faster in familiar surroundings with the right support in place.
Planning to go home
As soon as you’re well enough, we’ll begin planning for your discharge. You’ll be involved in all decisions about your care and what happens next.
Before you leave, we’ll make sure you:
- Have the information you need about your condition and ongoing care
- Understand your medications and how to take them
- Know about any follow-up appointments, rehabilitation, or support services
- Receive help from dietitians, occupational therapists, physiotherapists, or a social worker if required
We’ll also support you with any extra help you may need at home, including health and social care services.
Remember: Ask your nurse for any valuables you handed in for safekeeping, as well as any medical certificates you may need before you go.
Your medications
When you’re discharged, you’ll be given a supply of any medications you still need, along with:
- A green bag containing your medication
- A discharge letter detailing your treatment and prescribed medicines (your GP will also receive a copy)
Once home, book an appointment with your GP to arrange ongoing prescriptions and to discuss your continuing care. Make sure to do this before your medication runs out.
Getting home
You’ll need to make your own arrangements for getting home from the hospital. You can:
- Arrange for a friend or family member to collect you
- Travel by taxi (staff can provide contact details for local taxi services)
- Use hospital-arranged transport if it’s agreed that you’re eligible (this will be organised by the team if needed)
