As you prepare to leave the hospital, use this checklist to make sure everything is in place for a smooth transition home. If you need any help with these arrangements, please speak to the ward staff—they’re here to assist you.
- Do you know your planned discharge date?
- Have you informed your family or carers about when you’ll be going home?
- Have you arranged how you will get home?
- Is there someone available to take you home?
- Will your home be warm and comfortable on arrival? Ask someone to switch on the heating beforehand, aiming for at least 18°C.
- Do you have suitable outdoor clothing for your journey home?
- Do you have your house keys or another way to get into your home?
- Is there enough food and drink available at home? Consider asking someone to stock up for the first few days.
- Do you have all your personal belongings and valuables (e.g., glasses, hearing aids) with you?
- Do you have the contact details for your GP surgery to arrange follow-up appointments or medication reviews?
- Do you have all the equipment, dressings, or aids you need at home?
- Have you received your discharge letter and any other important documents such as outpatient appointment details or patient information leaflets?
Planning extra support at home
It’s important to make sure you’ll have the right support once you’re home. Please consider the following:
- Will you or your carer be able to manage daily tasks when you return?
- If you live alone, can a friend or relative check your home is ready and contact you shortly after your arrival?
- Do you anticipate any housing challenges after discharge?
- Will your home require adaptations to meet any new needs?
- Are you at risk of homelessness now or soon after leaving hospital?
- Will you need help with personal care?
- Will you require assistance with shopping, cleaning, meal preparation, or laundry?
- Would you benefit from services such as meals on wheels?
If you have any concerns or need help with any of the above, please discuss them with the ward staff or a social worker before you leave. We want to support you in making your return home as safe and comfortable as possible.
